MM/ HM/ MP – Medicare Masterpiece HMO and HMO-POS Members are now required to obtain a PCP REFERRAL for an office visit to an in-network specialist*.   The REFERRAL can be submitted via the website OR by sending a PCP Fax Referral Form to Universal.
