Program Requirements:

• IntegraNet Physician
• Individual Loss Ratio of 60% or less (10 yr)
• Program Loss Ratio of 60% or less
• Board Certified or equivalent
• ECFMG Certified (if applicable)
• Signed program participation agreement with IntegraNet
